For patients with a GP Management Plan and Team Care Arrangement.
What is covered:
✓ Up to 5 allied health sessions per calendar year under a CDM plan
✓ Exercise physiology sessions covered with a valid referral
For patients — how to access:
→ Ask your GP for a GP Management Plan and Team Care Arrangement
→ Bring your referral to your first appointment at PerforMotion
→ Call PerforMotion first if you are unsure whether you qualify
For referrers — how to refer:
→ Complete the Medicare referral form above
→ Or fax and email referrals directly to admin@performotion.net
→ Remind your patient to bring any imaging or reports to their first appointment
For current and former Australian Defence Force members with a DVA health card.
What is covered:
✓ Exercise physiology covered for eligible Gold and White card holders
✓ Prior approval may be required for some conditions
✓ PerforMotion bills DVA directly — no out-of-pocket cost for eligible clients

✓ Self managed, and agency managed participants welcome
✓ Programs delivered via the Lumin app and tracked against NDIS goals
✓ VALD performance testing available to support progress reporting
✓ Progress reports and outcome data provided on request
